As stated, while we don't need payment information, we do need to identify you - medically and a way to contact you.

1. Your email address - so we can contact you
2. Your age - it affects what treatments can be used
3. Your gender (are you male or female)- it affects what treatments can be used because there is a real biological (and medical) difference between men and women.
4. Your Blood Type - it affects what treatments can be used
5. Your ancestory (African, Greek, Irish, etc.) - it affects what treatments can be used
6. Geographical information (where you live) - if there is a health issue for a particular area with this information we can see and warn you.
